You will need:

1. A complete copy of the AFA subcontractor agreement (which you can download here)
2. Your federal tax id
3. A copy of your liability insurance to upload
4. A copy of you worker's compensation to upload
5. A copy of you license to upload

The subcontractor application is intended for new subcontractors to AFA. If you are currently providing services for AFA, please donot submit an application.It should take about 5 minutes to complete the application
